Rabbit Eye Health: Conditions, Signs + When to See a Vet

Rabbit eyes are not small dog eyes. They are anatomically distinct, behave differently under disease, and require species-specific knowledge to assess and treat correctly. Most general practice vets have limited experience with rabbit ophthalmology - which is exactly why Dr. Jennifer Sheahan's Vet Set talk at BCB covered so much ground that rabbit owners rarely hear from a single source.

This article captures the key points from that talk: the anatomy that makes rabbit eyes unique, the conditions most commonly seen in practice, and what owners actually need to know before something goes wrong.

Rabbit Eye Anatomy: Why It Matters

Understanding what's normal in a rabbit eye is the only way to recognize what's not.

Lateral eye placement
Rabbit eyes sit on the sides of the head - not the front. This is a prey animal adaptation. Lateral placement gives rabbits an almost panoramic field of view, allowing them to detect a predator approaching from nearly any angle. Their only blind spot is directly in front of and below the chin - a very small area. Compare this to predator species like cats, whose forward-facing eyes provide excellent depth perception for pouncing but a much narrower field of view overall.

Large corneas
The cornea makes up approximately 30% of the globe - a significantly larger proportion than in most mammals. This gives rabbits excellent wide-angle vision but also means the cornea is more exposed and more vulnerable to injury.

The merangiotic fundus
The back of a rabbit's eye has a distinctive structure called a merangiotic fundus - the blood vessels radiate outward from the optic nerve like angel wings. This is completely normal in rabbits and looks dramatically different from the fundus of a dog or cat. The optic nerve in rabbits also has a cupped, depressed appearance that in other species would indicate chronic disease or glaucoma - in rabbits, this is simply normal anatomy. A veterinarian unfamiliar with rabbit eyes may misread this as pathology. This is one of the reasons rabbit-savvy ophthalmology expertise matters.

Rabbits also lack a tapetum - the reflective layer behind the retina that makes cats' and dogs' eyes shine in the dark. This is not a deficiency. Rabbits are prey animals, not nocturnal hunters, and have no evolutionary need for that light amplification system. The absence of eyeshine in your rabbit is normal.

The optic nerve and focal point
The optic nerve in rabbits sits above the horizontal axis of the eye - which means the focal point is directed downward. This makes biological sense: grass and ground-level food sources are what a rabbit needs to see most clearly. The anatomy of the eye reflects the animal's ecological reality.

Blinking rate
Rabbits blink only 10 to 12 times per hour - not per minute. This is significantly less than most mammals and puts them at increased risk for dry eye, corneal exposure, and surface disease. They also lack a menace response - they will not reflexively blink when something moves toward their eye. Both are normal but clinically significant. A rabbit sitting with eyes wide open is not necessarily comfortable or healthy - it may simply be that their baseline blink rate masks early surface disease.

Nuclear Sclerosis vs. True Cataracts

One of the most common points of confusion for rabbit owners: cloudiness in the eye does not always mean cataracts.

Nuclear sclerosis is an aging change that causes the lens to develop a pearly, slightly hazy appearance. Dr. Sheahan described it as the rabbit equivalent of needing bifocals - a depth perception change, not a vision loss. A rabbit with nuclear sclerosis may pause before a jump they used to make confidently, or seem slightly uncertain navigating familiar spaces. They are not blind. They are aging. Nuclear sclerosis does not require treatment.

True cataracts are a complete whiting out of the lens. As Dr. Sheahan put it: driving with snow on your windshield. The lens, normally transparent and shaped like an M&M (the candy), becomes fully opaque and blocks light from reaching the retina. This is a meaningful visual impairment and in many cases a surgical one.

The distinction matters enormously. If you look at your rabbit's eyes tonight and see a slight haze or pearliness, that is almost certainly nuclear sclerosis. A complete, dense white opacity is a cataract. If you are unsure, that is what a veterinary ophthalmologist is for.

Cataract surgery in rabbits
Rabbit cataract surgery uses the same equipment as human cataract surgery - an ultrasound probe, not a laser, despite what most people assume. The probe breaks up the clouded lens and suctions it out. A rabbit-specific artificial lens is then placed, and when the lens capsule is stable enough, the result can be 20/20 vision.

Under an operating microscope, the eye is magnified 32 times - effectively the size of a television screen to the surgeon. The rabbit is positioned on its side under general anesthesia. At VEC, Dr. Sheahan works alongside a dedicated exotic veterinary anesthesiologist - the ophthalmologist focuses exclusively on the eye while the exotic vet manages the anesthesia. This level of specialization matters given how sensitive rabbits are to anesthesia.

When bilateral cataracts are present, both eyes can be addressed in a single procedure - the rabbit is simply repositioned to the other side once the first eye is complete.

Grooming and Eye Health: What Your Groomer Is Trying to Tell You

Long-haired rabbits, particularly Teddywidders, Angoras, and other wool breeds, are at significantly elevated risk for fur-related eye complications. This is not an aesthetic issue. It is a medical one.

Fur that grows toward the eye and is left untrimmed mats against the corneal surface. Matted fur does not just irritate - it creates a physical barrier that traps discharge, holds moisture against the skin, causes fur loss, and over time contributes to corneal ulceration and chronic infection. By the time a rabbit reaches a groomer or a vet with a cement-hard mat wedged against the eye, the damage is often already underway.

The tension between "I want my rabbit to look cute" and "this cut is medically necessary" is one grooming professionals navigate constantly. A short trim around the eyes is not a style choice - it is the difference between a manageable maintenance situation and a very expensive corneal patch surgery for an ulcer that penetrated too deep.

Dr. Sheahan's message was direct: listen to your groomer. Groomers often notice early eye changes - asymmetry, discharge, fur matting toward one eye - before owners do, because they are working up close with hands on the animal. When a groomer flags something, that is a referral, not an opinion. Take it seriously.

Eyelid Conditions

Entropion
Entropion is the inward rolling of the eyelid, causing the eyelid margin or lashes to rub directly against the corneal surface. This is painful and will cause corneal ulceration if untreated. Correction is surgical.

Cherry Eye (Prolapsed Nictitans Gland)
Cherry eye appears as a pink or red protuberance at the inner corner of the eye - the gland of the third eyelid has prolapsed out of its normal position. It looks alarming but is not immediately dangerous. The concern is long-term: left untreated, the exposed gland dries out, becomes irritated, and begins rubbing directly on the corneal surface.

The gland contributes to tear production and should never simply be removed. The correct surgical approach is the Morgan pocket technique - incisions are made on each side of the gland and it is sutured back into position so it can continue producing tears without contacting the eye. Outcomes are favorable when performed correctly.

Pterygium
A pterygium is an overgrowth of conjunctival tissue that grows across the cornea from the periphery toward the center. It is not painful but progressively impairs vision as coverage increases. Treatment is surgical removal. It can look like corneal scarring from a chemical burn - the distinction is made by determining whether the tissue is free-floating over the cornea (pterygium) or adhered to it (scar tissue).

Corneal Ulcers

A corneal ulcer is a defect in the corneal surface - ranging from a superficial scratch to a deep penetrating wound. Given how infrequently rabbits blink, they are at higher baseline risk for corneal surface injury and slower healing than other species. Signs include squinting, excessive tearing, redness, pawing at the eye, cloudiness, or a visible defect on the corneal surface.

Diagnosis requires fluorescein staining (a dye that highlights corneal defects) at a veterinary exam. Treatment depends on depth: superficial ulcers may resolve with antibiotic eye drops; deep ulcers may require surgical repair.

**Critical Medication Warning: Amoxicillin-clavulanate (Clavamox) - one of the most commonly prescribed antibiotics in dogs and cats - is toxic to rabbits. Never use leftover dog or cat medications on a rabbit. If your rabbit needs antibiotic eye drops, confirm with a rabbit-savvy veterinarian that the formulation is safe for your species.

Conjunctivitis and Epiphora (Excessive Tearing)

Chronic eye discharge, tearing, or wet fur under the eye in a rabbit usually indicates a problem with the nasolacrimal duct - the drainage channel that carries tears from the eye to the nose. When this duct is blocked, tears overflow onto the face, eventually causing skin irritation and fur loss.

Common causes:

  • Dental disease: the root of the upper incisor sits immediately adjacent to the nasolacrimal duct. Tooth root disease can compress or obstruct the duct. Any rabbit with chronic eye discharge should have a thorough oral exam.

  • Foreign body (seeds, hay particles, dust)

  • Bacterial infection

  • Scarring from chronic untreated infection

Lops are at higher risk due to their compressed skull anatomy, which narrows the duct further than in upright-eared breeds.

First line of treatment is usually antibiotic eye drops. If that doesn't resolve it, duct flushing (passing a blunt flexible catheter through the duct opening and irrigating with saline) can clear a mucus plug or debris. This is typically performed with the rabbit awake. If the duct has been obstructed long enough to scar closed, flushing is no longer possible and ongoing management becomes the goal: keeping the area clean, preventing matting, and protecting the skin around the eye.

For chronic discharge cases, a dilute betadine eyewash (approximately 1:50 dilution with purified water, not commercial multi-ingredient formulations) can be used to keep the periocular area clean and reduce secondary skin breakdown. This is something to discuss with your ophthalmologist directly, not a home remedy to improvise.

Uveitis and EC-Related Eye Disease

Uveitis (inflammation of the uveal tract) causes the eye to appear red, cloudy, and painful. In rabbits, the most common infectious cause is Encephalitozoon cuniculi (EC).

EC can infect the lens of the eye in utero, during fetal development when the lens is briefly accessible to the parasite. The parasite replicates inside the lens and eventually causes phacoclastic uveitis - the lens ruptures, releasing inflammatory proteins into the front of the eye and triggering a severe inflammatory response. This typically appears in younger rabbits and usually affects one eye. Cataracts often develop as a secondary consequence. A white granuloma may be visible in the pupil - this is inflammatory tissue, not pus.

Pasteurella can also cause uveitis via bloodborne spread to the eye.

Treatment involves anti-inflammatory medications and fenbendazole for the underlying EC infection. Eye removal is considered when the eye is both blind and painful and cannot be managed medically.

Orbital Disease

When something behind the eye pushes the eyeball forward out of the socket, this is orbital disease. Signs include a protruding eye, third eyelid swelling, and abnormal eye position.

Common causes:

  • Tooth root abscess: the most common cause, especially in lops and rabbits with dental disease history. A severe tooth root infection can push into the orbital space and displace the eye. Once the underlying infection is resolved, the rabbit can live a normal life.

  • Lymphoma: the most common ocular cancer in rabbits. Ocular lymphoma typically affects both eyes and is associated with systemic signs including reduced appetite and involvement of the spleen and liver. Prognosis is guarded.

CT imaging and fine needle aspirate behind the eye are the primary diagnostic tools. Single eye involvement is more likely to be infection; bilateral involvement raises concern for lymphoma.

Eye Removal (Enucleation)

The guiding principle in veterinary ophthalmology is straightforward: a blind but comfortable eye does not need to come out. Eye removal is considered only when the eye is both blind and painful with active tearing, uncontrolled pressure, globe swelling, or abscess that cannot be managed with medication.

Rabbits adapt well to monocular vision, particularly when they have a bonded companion to follow. The surgery, when performed by an experienced ophthalmologist alongside an exotic anesthesiologist, is well-tolerated in healthy rabbits.

When to See a Rabbit-Savvy Eye Specialist

Your primary exotic vet is an excellent first stop for most eye concerns. Referral to a veterinary ophthalmologist is appropriate for:

  • Any corneal ulcer not resolving with initial treatment

  • Cataracts causing meaningful vision impairment

  • Cherry eye requiring surgical correction

  • Any eye that is red, painful, and not responding to treatment

  • Suspected orbital disease or mass

  • Any situation where your primary vet is uncertain about what they're seeing
